What You'll Do8
- 1Oversee and direct patient care services to ensure high-quality hospice care
- 2Manage visit scheduling including reassigned missed declined or rescheduled visits using clinical judgment
- 3Triage incoming phone referrals and medical concerns professionally
- 4Lead or facilitate Interdisciplinary Group meetings presenting patient updates reviewing charts and coordinating care plans
- 5Serve as mentor trainer and clinical support to associates focusing on development performance and compliance
- 6Ensure all care delivery meets federal state and local regulatory standards and company policies
- 7Support performance improvement initiatives quality assessments and operational efficiencies
- 8Develop and implement strategies to streamline processes and enhance team effectiveness
Requirements12
- 1Current RN license in the state of employment
- 2Graduate of an accredited nursing program
- 3Minimum of 3 years direct patient care experience including 2 years in hospice or home-based care
- 4Demonstrated ability to apply hospice principles industry standards and regulatory compliance Medicare Medicaid JCAHO ACHC
- 5Knowledge and compassion for terminally ill patients and families
- 6Ability to lead diverse care team adapting to emotional and clinical needs
- 7Strong interpersonal organizational and leadership skills
- 8Competency in clinical documentation and technology systems
- 9Bachelor of Science in Nursing preferred
- 10Previous leadership team management or supervisory experience in hospice or home health
- 11Familiarity with EMR systems and telehealth platforms
- 12Certification in Hospice and Palliative Nursing CHPN preferred
