RN Case Manager – Home Health Indianapolis
Overview
We seek an experienced compassionate Registered Nurse (RN) Case Manager to deliver in-home care across Indianapolis. This role provides autonomy flexibility and the chance to build lasting patient relationships while managing your own caseload.
What You'll Do9
- 1Perform comprehensive in-home patient assessments including OASIS
- 2Develop implement and evaluate individualized care plans
- 3Provide skilled nursing care and treatments
- 4Educate patients and families on care plans and disease management
- 5Coordinate care with physicians and interdisciplinary team members
- 6Supervise LPNs and ancillary staff as needed
- 7Complete timely and accurate electronic documentation
- 8Ensure compliance with all regulatory and agency standards
- 9Participate in team meetings and quality improvement QAPI
Requirements6
- 1Active Indiana RN license
- 2ASN or BSN from an accredited nursing program
- 3Home health experience preferred including OASIS
- 4Experience with electronic health records preferred
- 5Strong clinical communication and organizational skills
- 6Ability to work independently in a home health setting
