RN Case Manager Hospice - Phoenix Home Care
Overview
You will own hospice care coordination for patients in their homes across Overland Park and surrounding areas, managing caseloads of 15-20 patients. You will guide families through care decisions and collaborate with physicians and the hospice team to adjust plans of care. What You'll Do9
- 1Provide hands-on hospice care to patients in their homes, managing pain and symptoms with RN expertise.
- 2Guide families through care decisions with empathy and education, ensuring they understand each step.
- 3Collaborate with physicians and the hospice team to adjust plans of care based on patient needs.
- 4Conduct thorough assessments and ongoing evaluations, documenting changes in condition.
- 5Support patients' physical, emotional, and comfort needs at every stage, from initial visit to end-of-life.
- 6Coordinate with Kansas or Compact RN license requirements to ensure compliance.
- 7Drive to patient homes across the service area, using your own vehicle with valid insurance.
- 8Educate patients and families on disease progression and comfort measures, reducing anxiety.
- 9Partner with chaplains, social workers, and aides to deliver holistic care, reflecting Christ-like values.
Requirements6
- 1Active Kansas or Compact RN license.
- 2Hospice experience preferred, but we will train the right RN.
- 3Valid driver's license and auto insurance for home visits.
- 4Ability to assess and respond to patient needs in a home setting.
