Hospice RN Case Manager Job in Kansas City
Overview
You will own end-of-life care for hospice patients in their homes across Kansas City, managing a caseload of 15-20 patients. Your days center on comprehensive assessments, care plan development, and coordination with the interdisciplinary team. What You'll Do9
- 1Assess hospice patients comprehensively within 24 hours of admission and develop individualized care plans in EPIC EMR.
- 2Coordinate care with the interdisciplinary team, including physicians, social workers, and chaplains, through weekly IDT meetings.
- 3Administer medications and treatments per physician orders, including IV infusions and pain management protocols.
- 4Educate patients and families on symptom management, medication schedules, and end-of-life care processes.
- 5Monitor and update care plans after each visit based on reassessment of physical, emotional, and spiritual needs.
- 6Provide emotional and clinical support to patients and caregivers during transitions and crisis moments.
- 7Document all visits and phone calls in HospiceMD within 24 hours, ensuring compliance with regulatory standards.
- 8Supervise hospice aides and LVNs, delegating tasks and verifying quality of care.
- 9Participate in on-call rotation to address urgent patient needs evenings and weekends.
Requirements8
- 11+ years of RN experience in hospice, home health, or a related field.
- 2Active RN license in the state of Missouri.
- 3BSN or CHPN certification preferred.
- 4Current CPR/BLS certification.
- 5Valid driver's license, auto insurance, and reliable transportation.
- 6Strong interpersonal and clinical assessment skills.
- 7Ability to work independently and within a team.
- 8Pass pre-employment drug screening, including marijuana.
