RN Case Manager, Hospice & Home Health
Overview
You will own patient admissions, assessments, and care coordination as an RN Case Manager in hospice and home health across the Overland Park area. You work independently in the field, joining an interdisciplinary care team to deliver end-of-life care. This role demands a licensed RN with 1+ years of experience, and offers a $7,500 sign-on bonus and full benefits.
What You'll Do7
- 1Admit patients by completing initial assessments and developing individualized care plans with the care team.
- 2Coordinate care across disciplines, scheduling visits and communicating with physicians and family members.
- 3Perform ongoing patient assessments and modify plans to reflect changing needs and goals.
- 4Discharge patients per hospice guidelines, ensuring documentation meets regulatory standards.
- 5Pronounce death when required, providing support to families and staff during the process.
- 6Document all visits and patient interactions accurately using EHR systems.
- 7Travel to patient homes and facilities within the service area, managing your own schedule.
Requirements6
- 1RN license in the State of Kansas
- 21+ years of experience as an RN
- 3Hospice or home health experience preferred
- 4Effective written and verbal communication skills
- 5Reliable transportation and ability to travel daily
- 6Clean background, drug screen, and driving record
