Post Acute Liaison Home Care, Healthcare Sales & Coordination
Overview
Owns referral coordination for Banner Homecare services and drives patient pathways across hospital and clinic settings. 80 percent onsite, 20 percent field engagement to align patients with post-acute services such as specialty pharmacy, HME, and home infusion therapy. Collaborates with providers, care coordination staff, and business development teams to educate physicians and staff on Banner’s comprehensive post-acute offerings. This role shapes patient care outcomes through clinical knowledge and strategic influence.
What You'll Do7
- 1Build relationships with hospital, skilled nursing, and discharge planning staff to increase admissions and meet targets
- 2Develop a Homecare presence within facilities and deliver education programs to referral sources
- 3Maintain positive relationships with referral sources and coordinate communication between patients, families, and providers
- 4Function as liaison between referral sources and Banner Homecare ensuring appropriate clinical alignment and post-acute resources
- 5Ensure effective cross-functional communication with program staff and participate in weekly business development meetings
- 6Assist with staff and physician orientation and contribute to performance improvement activities
- 7Work independently under general supervision while coordinating care across multiple facilities and specialty practices
Requirements8
- 1Must have degree in nursing, healthcare or related field
- 2Three or more years of related sales experience and education in healthcare services
- 3Strong knowledge of Medicare eligibility and governmental regulations
- 4Excellent organizational skills and ability to manage time and resources independently
- 5Effective negotiation, customer service and interpersonal relations skills
- 6Ability to communicate effectively verbally and in writing and through common computer software
- 7Arizona Fingerprint Clearance Card at time of hire and maintain
- 8Home health and/or hospice experience preferred
