Case Manager Lead, Behavioral Health & Integrated Care
Overview
Own the integrated care planning process for members with chronic conditions guiding the ISP from inception to review. Influence care coordination across behavioral health, medical, and dental providers at scale in Arizona. Collaborate with the integrated care team, families, and community resources to ensure services align with recovery goals. This role emphasizes mentorship, documentation accuracy, and progress-based plan adjustments.
What You'll Do5
- 1Mentor and train new Case Managers on EMR-Next Gen Tele-Health and shadowing practices
- 2Coordinate with behavioral health, medical, and dental providers to implement the Integrated Service Plan in a timely manner
- 3Develop and maintain the Integrated Service Plan and adjust services based on member progress
- 4Maintain comprehensive clinical records including assessments, services provided, and discharge planning
- 5Support progress reviews and ensure documentation complies with regulatory and organizational standards
Requirements5
- 1Bachelor’s degree in a behavioral health field with 2 years of relevant experience or Bachelor’s degree in a non-behavioral health field with 4 years of relevant experience
- 2High school diploma with 6 years of relevant behavioral health work experience
- 3Level 1 fingerprint clearance card or ability to obtain within 8 weeks
- 4Arizona driver’s license and vehicle with minimum insurance and ability to meet driving requirements
- 5Pre-Employment Criminal Background and Drug Testing required
