Case Manager, Behavioral Health & Recovery Services
Overview
Owns care coordination across the full continuum of recovery from PHP/IOP through outpatient and sober living at a Joint Commission–accredited center. You will develop individualized care plans, collaborate with a multidisciplinary team, and ensure smooth transitions while maintaining HIPAA compliant documentation. This role sits at an onsite outpatient location in Phoenix, focused on patient-centered outcomes in a collaborative, mission-driven environment. Your work directly impacts client recovery journeys and supports a growing behavioral health program.
What You'll Do7
- 1Build comprehensive assessments across medical, social, behavioral health, and psychosocial domains to inform care plans
- 2Design and maintain individualized discharge plans with clinical treatment, family involvement, and community resources
- 3Coordinate with therapists, Clinical Director, Medical Director, and support staff to deliver cohesive care
- 4Facilitate discharge planning by arranging step-down care, sober living placement, or outpatient services
- 5Maintain accurate EMR/EHR documentation in compliance with HIPAA and 42 CFR Part 2
- 6Educate clients and families on treatment options and next steps
- 7Coordinate with referral sources, employers, legal contacts, and community agencies for ongoing support
Requirements7
- 1Bachelor’s degree in Social Work, Psychology, Counseling, or related field
- 2Master’s degree preferred but not required
- 3Experience in behavioral health, substance use treatment, or case management preferred
- 4Familiarity with medical terminology and case management principles
- 5Knowledge of managed care processes, health insurance policies, utilization management, and discharge planning
- 6Proficiency with EMR/EHR systems and Microsoft Office
- 7Knowledge of ASAM criteria, HIPAA, and 42 CFR Part 2 is a strong plus
