Care Coordination Case Manager, Behavioral Health & Healthcare Navigation

What You'll Do7
- 1Coordinate member care and communicate needs across clinical teams to improve access and outcomes
- 2Assist members with medical services, referrals, appointments, and healthcare navigation to reduce delays
- 3Maintain accurate records and documentation handling sensitive information with confidentiality
- 4Support check-ins, scheduling, and other care coordination activities to sustain engagement
- 5Help with telehealth and tele-psych appointments to expand access
- 6Ensure compliance with HIPAA, OSHA, and organizational requirements
- 7Provide compassionate support and encourage member engagement in services
Requirements9
- 1High school diploma or GED
- 2Behavioral Health Technician designation or related experience
- 3Knowledge of patient care, medical terminology, and healthcare documentation
- 4Strong communication, organization, and problem-solving skills
- 5MS Office and electronic health record systems proficiency
- 6CPR/First Aid certification and crisis de-escalation training
- 7At least 1-2 years in healthcare, behavioral health, care coordination, or case management
- 8Bilingual (Spanish) preferred
- 9Must be at least 21 years old
