Case Management Navigator, Behavioral Health & Community Resources
Overview
Own outreach, engagement and case management for individuals with serious mental illness at scale within a non profit health system. You will complete assessments and service plans, coordinate care across behavioral health medical and community providers, and monitor progress toward recovery. Be part of a collaborative team focused on compassionate, person centered care and crisis intervention.
What You'll Do10
- 1Build outreach to unengaged members and coordinate care with families and providers to implement ISP based services
- 2Design and document progress toward recovery through regular communications and comprehensive records
- 3Lead coordination of care across behavioral health medical dental and community resources ensuring all services align with the ISP
- 4Ship brief assessments and service plans to focus on inter agency coordination and continued outreach
- 5Own transportation support and direct case management tasks while maintaining accurate clinical records
- 6Drive continuous evaluation of care level and make recommendations to supervisor
- 7Coordinate with clinical team to explain options and articulate advantages and disadvantages
- 8Collaborate with families or significant others to implement effective service plans and ensure compliance with agency policies
- 9Maintain documentation including assessments service delivery and discharge planning
- 10Support quality management utilization review and agency fundraising activities
Requirements5
- 1Bachelor’s degree preferred
- 2High School Diploma or GED required
- 3Combination of education and experience meeting BHT requirements per ADHS
- 4Minimum one year of community behavioral health case management experience
- 5Experience conducting outreach with seriously mentally ill or homeless populations
