Case Manager 3, Developmental Disabilities
Overview
Own and coordinate individualized supports for Arizonans with developmental disabilities at scale within the DES DDD team. You will drive on-site visits, develop ISP plans, and connect members with community resources. What You'll Do10
- 1Build and maintain member service plans and ISPs with timely progress notes and referrals
- 2Drive on-site visits to homes, schools, and program sites to discuss service needs and supports
- 3Develop, write, review, and monitor ISP documentation in accordance with mandated timelines
- 4Research community resources to meet identified needs and inform members of options
- 5Facilitate meetings and program visits to support member and family choices and crisis support
- 6Collaborate with community agencies to provide assistance and support during crises
- 7Mentor and train new and existing team members in the unit
- 8Lead a team of case managers in the absence of the supervisor
- 9Conduct referrals for assessed services and ensure compliance with HIPAA and state/federal guidelines
- 10Maintain confidentiality and work effectively with diverse teams
Requirements9
- 1Arizona licensed Registered Nurse in good standing OR Bachelor Degree in Social Work OR Bachelor’s degree in Psychology, Special Education or Counseling with at least one year of case management experience
- 2Two years of case management experience for elderly, disabled, or SMI populations for certain tracks
- 3One year of case management experience for elderly, disabled, or SMI populations for other tracks
- 4Valid driver license and ability to drive as essential function
- 5Ability to pass background checks and Child Protective Services Central Registry checks
- 6E-Verify employment eligibility verification
- 7Arizona Level 1 Clearance Card or ability to obtain
- 8Strong written and verbal communication skills
- 9Ability to work independently and within diverse teams
