Overview

As a Case Manager at Central Arizona Shelter Services, you will own the full client journey from intake to housing stability, working directly with individuals experiencing homelessness. What You'll Do10

  • 1Conduct in-depth client interviews and orient individuals to the program, assessing strengths, needs, barriers, and goals to co-create individualized case plans aligned with Social Determinants of Health.
  • 2Provide referrals, resource information, and guidance to support client progress, updating case plans regularly to reflect changes and evolving needs.
  • 3Hold regular follow-up meetings to review progress, monitor system notes from other departments, and gather additional information to keep clients on track toward their goals.
  • 4Connect clients to essential services and benefits, including clothing, transportation, identification documents, healthcare, and behavioral health, using established and identified resources.
  • 5Collaborate with housing providers to expedite placements and ensure long-term housing stability, supporting job readiness by evaluating employment potential, reviewing job listings, and assisting with applications and resumes.
  • 6Assist clients with completing forms, applications, and paperwork for housing, benefits, and services, and provide walk-up assistance for urgent needs.
  • 7Transport clients to appointments when other options are not available, and serve as an advocate by championing client needs and ensuring their voice is central in service planning.
  • 8Enter client information into HMIS promptly after intake, maintain complete and accurate notes in client records before each shift ends, and review updated case notes daily to ensure continuity of services.
  • 9Close HMIS files promptly after client exit and provide success stories and positive progress information to leadership.
  • 10Maintain respectful relationships with partner agencies, collaborate with team members for a holistic approach, and escalate urgent issues to leadership in a timely manner.

Requirements9

  • 1Associate’s degree in social or behavioral science or related field and 1+ years of social services experience, or equivalent relevant work experience.
  • 2Working knowledge of community resources and ability to obtain a State of Arizona Level One Fingerprint Clearance Card.
  • 3Intermediate to advanced proficiency in Word, Excel, and Outlook with highly accurate data entry skills.
  • 4Arizona Driver’s License and a clean driving record for the past 3 years, free of traffic/moving violations, suspensions, or revocations.
  • 5Pass behind-the-wheel certification within 90 days of employment.
  • 6CPR-First Aid certification within 90 days of employment.
  • 7Naloxone (Narcan) training within 90 days of employment.
  • 8Non-violent crisis de-escalation certification within 90 days of employment.
  • 9Preferred: Experience with HMIS (Homeless Management Information System).
