Case Manager, Social Work & Trauma-Informed Care
Overview
A Case Manager will support high-risk youth and survivors in the Victim and Survivor Services program in San Diego. You will coordinate housing, employment, education, health, and related services for up to 90 days or longer, with on-call duties and intensive follow-up. You will join a team focused on trauma-informed care, collaboration with Probation, CWS, health providers, and community partners. This role is onsite in San Diego and offers benefits and PSLF eligibility.
What You'll Do10
- 1Build and maintain accurate documentation and ensure contract compliance for records
- 2Coordinate care across Probation Officer, CWS Social Workers, Health Care Providers, Therapists and families
- 3Engage youth in services with intensive short-term case management and 24hr follow-up on referrals
- 4Drive outreach activities to increase community awareness and develop group curricula
- 5Lead crisis intervention, outreach, and referrals in collaboration with partners
- 6Transport youth to meetings, appointments, court, and other activities as needed
- 7Triage needs with the CSEC Response Team for eligible services
- 8Assist in developing and maintaining group curriculum and facilitate groups
- 9On-call availability to support service delivery 24/7 when scheduled
- 10Seek partnerships with low-income housing providers and monitor service partner eligibility
Requirements10
- 1Bachelor's degree in social work or related field preferred
- 22-5 years’ experience with high-risk adolescents or Transitional Aged Youth and those affected by commercial sexual exploitation or trafficking considered in lieu of education
- 3Lived experience with commercial sexual exploitation or trafficking required
- 4Bi-lingual in Spanish preferred
- 5Experience in crisis intervention, group facilitation, documentation and case management preferred
- 6Excellent verbal and written communication skills
- 7Willingness to develop trauma-informed care knowledge
- 8Must complete or be willing to complete 40-hour Domestic Violence counselor training, Campbell Danger Assessment, Mental Health First Aid, Seeking Safety, and Motivational Interviewing trainings
- 9Computer literacy in Outlook, Word, Excel; valid CA driver’s license and insured vehicle; CPR and First Aid certification
- 10Clearances including TB test, fingerprints, background checks, and drug testing; available for on-call hours (Mon-Sun 24/7)
