Overview
The Case Manager provides client-centered, trauma-informed care to survivors of crime, supporting their safety, healing, and self-sufficiency. This role assesses needs, develops care plans, and coordinates services for individuals and families, often in crisis situations. You will work with a team of advocates and clinicians, using Spanish and English to serve diverse communities. The position is onsite in Denver and part of the Victim Supportive Services program, offering comprehensive care without requiring police involvement.
What You'll Do10
- 1Assess client needs using a comprehensive tool addressing social determinants of health, then develop tailored care plans.
- 2Facilitate trauma-informed interventions and harm reduction strategies, adapting to each client's unique situation.
- 3Provide psychoeducation on mental health and substance misuse to help clients understand and manage their conditions.
- 4Develop safety plans for clients, prioritizing their immediate and long-term well-being.
- 5Respond to crises with de-escalation techniques and ensure client stability during high-stress situations.
- 6Participate in community outreach events across diverse settings to connect with underserved populations.
- 7Coordinate internal and external referrals, linking clients to essential resources and services.
- 8Maintain accurate and timely documentation of all client interactions, following HIPAA guidelines.
- 9Build partnerships with community organizations to enhance service access and support.
- 10Support agency fundraising and outreach efforts, representing Servicios de La Raza to the public.
Requirements9
- 1Valid Driver's License and reliable transportation for client visits and outreach.
- 2High School Diploma or equivalent; relevant experience may substitute for education.
- 3Bilingual English-Spanish proficiency to serve diverse client populations.
- 4Experience with trauma-informed care, crisis intervention, and harm reduction with survivors of crime.
- 5Knowledge of social determinants of health and case management practices.
- 6Commitment to equity, cultural relevance, and serving communities of color.
