What You'll Do8
- 1Completes accurate documentation promptly following department standards
- 2Assists in preparing required reports
- 3Conducts initial and ongoing assessments of client health needs

- 6Provides basic and intensive individual support including referrals
- 7Accompanies clients to housing appointments and social services
- 8Performs other duties as assigned
Requirements6
- 1Bachelor's degree in Social Science field or related discipline preferred
- 2One year human services experience with high risk medically underserved populations
- 3Multi lingual abilities English Spanish Arabic Somali
- 4Valid California driver's license and automobile
- 5Minimum auto insurance coverage $100000
