What You'll Do8

  • 1Serve as primary clinician and coordinator for an assigned caseload, delivering assessments, mental health evaluations, and family-individual-group therapy.
  • 2Build care plans with clients and team members, setting measurable goals for symptom management and daily living skills.
  • 3Lead the ICM step-down team, overseeing caseload tracking, documentation compliance, and treatment fidelity.
  • 4Coordinate referrals to next-level care, advocate for clients, and partner with physical health providers for integrated care.
  • 5Facilitate pre-placement visits and provide transportation support as needed.
  • 6Handle crisis intervention and participate in the on-call rotation to ensure 24/7 client safety.
  • 7Complete accurate and timely documentation, meeting minimum billable requirements for direct services.
  • 8Maintain strict confidentiality of client care and uphold ethical standards in all interactions.

Requirements8

  • 1Master's degree and CA BBS registration or license (MFT, LCSW, LPCC) required.
  • 2Valid CA driver's license and current auto insurance per contract requirements.
  • 3Experience providing mental health and/or substance use disorder services preferred.
  • 4Cultural competency understanding required.
  • 5Bilingual preferred.
  • 6Knowledge of Microsoft Office and EHR software preferred.
  • 7Ability to provide field-based services and travel between offices/clients.
  • 8Physical capability to stand, sit, stoop, and lift heavy items as needed.
