Overview

Lead comprehensive intake, trauma-informed case management, and care coordination for high-needs youth and adults within a Queens supervised release program. What You'll Do10

  • 1Build and conduct comprehensive intake assessments using evidence-based tools to identify mental health, substance use, housing, and other psychosocial needs
  • 2Provide trauma-informed, person-centered case management to a caseload of high-risk participants
  • 3Collaborate with a multidisciplinary team to develop and implement individualized service plans
  • 4Support participants in navigating behavioral health, housing, public benefits, and legal services
  • 5Maintain regular in-person and phone contact to monitor progress, re-arrests, and court compliance
  • 6Create safety plans, provide supportive counseling, and encourage readiness for change
  • 7Prepare and present written progress reports to the court and advocate for participants' needs
  • 8Coordinate with defense attorneys, service providers, and community partners for continuity of care
  • 9Maintain accurate documentation in the agency case management system
  • 10Participate in ongoing training and reflective supervision to support professional growth

Requirements10

  • 1MSW or relevant master’s degree with two years of related case management experience
  • 2Knowledge of NYC Court Operations; preferred LMSW, LMHC, or LCAT licensure
  • 3Familiarity with Alternative to Detention (ATD) or Alternative to Incarceration (ATI) models
  • 4Working knowledge of treatment programs and referral options for housing, education, employment, and supports
  • 5Ability to exercise sound judgment and communicate clearly in writing and speech
  • 6Excellent time management and in-person and remote communication skills
  • 7Computer literate with adaptability to changing technology
  • 8Bilingual Spanish preferred
  • 9CASAC or CASCT encouraged to apply
  • 10Availability to work evenings and weekends as needed and travel within NYC boroughs
