Case Manager at CUCS in New York, NY onsite full-time, supporting The Hegeman 160-unit residence in Brownsville. What You'll Do10
- 1Build and update service plans and conduct ongoing assessments to monitor client progress
- 2Connect clients to healthcare, benefits, housing, employment, and community resources
- 3Provide crisis intervention, medication support, group facilitation, and daily living assistance
- 4Deliver services onsite and in the field including home visits and escorting clients to appointments
- 5Complete timely documentation and ensure compliance with funder standards
- 6Collaborate with multidisciplinary teams to address client needs and promote recovery and wellness
- 7Monitor and document progress, adjust plans as needed to achieve housing stability
- 8Engage clients in strengths-based, trauma-informed care approaches
- 9Support clients in navigating municipal and community resources
- 10Maintain accurate records and protect client confidentiality
Requirements7
- 1Education/Experience: HS Diploma + 2 yrs experience (CM I) up to Bachelor’s + 4 yrs experience (CM IV)
- 2College credits may substitute for experience
- 3Skills: strong communication, organization, crisis management; MS Office proficiency; bilingual preferred
- 4Physical/Field: ability to travel across NYC, escort clients on transit, lift up to 30 lbs
- 5Valid driver’s license may be required
