Staff Nurse Community Care, Home Health & Field Nursing
Overview
You will deliver community-based nursing care to patients in group and private homes, and to unhoused individuals across New York City. Under the direction of the supervising Assistant Director of Nursing, you will assess, plan, and evaluate care for complex cases including chronic disease, behavioral health, and wound care. You will work with a 600-person team at NYC Health + Hospitals/Community Care in Brooklyn, Manhattan, Queens, and The Bronx. This role stands out for its direct impact on underserved populations and collaboration with interdisciplinary teams. You will use OASIS documentation and evidence-based practices in every visit, with full autonomy in field settings.
What You'll Do10
- 1Perform initial evaluation visits and skilled nursing care per the physician's plan of treatment for patients with medical illnesses, chronic diseases, and behavioral health disorders.
- 2Develop and implement individualized care plans, revising them based on patient response and physician orders.
- 3Complete comprehensive assessments, including OASIS documentation, within 48 hours of each visit encounter.
- 4Deliver hands-on procedures: wound care including NPWT/VAC, PEG and Trach care, Foley insertion, JP/PleuRx drain care, and injections.
- 5Teach patients and families on disease management, medication administration, and self-care techniques.
- 6Coordinate with physicians, case managers, and interdisciplinary teams to ensure continuity and timely discharge planning.
- 7Supervise home health aides and other paraprofessional staff, ensuring proper documentation and care delivery.
- 8Document patient progress in clear, concise notes and maintain up-to-date records, including care plans and re-certifications.
- 9Respond to emergency situations and escalate clinical changes to the physician or non-physician provider as needed.
- 10Participate in team meetings, case conferences, and quality assurance performance improvement (QA/PI) activities.
Requirements8
- 1Valid New York State license and current registration as a Registered Professional Nurse.
- 2Current Basic Life Support (BLS) certification through the American Heart Association.
- 3For licenses issued on/after July 1, 2020: Bachelor of Science in Nursing or higher (or obtain within 10 years of initial licensure), per NYSED requirements.
- 41+ years of community health, home care, or public health nursing experience preferred.
- 5Ability to travel across all five boroughs via public transportation or private vehicle.
- 6Proficiency in OASIS documentation and electronic health records.
- 7Strong clinical assessment skills for acute and chronic conditions.
- 8Commitment to continuing education and professional development.
