- 1Provide specialized nursing care in homes and private residences following physician orders
- 2Conduct comprehensive assessments distinguishing physical and psycho-social needs
- 3Develop implement evaluate individualized care plans using OASIS documentation
- 4Record detailed progress notes maintain electronic files and obtain written orders promptly
- 5Supervise home health aides monitor patient conditions and report changes
- 6Teach patients families and allied staff disease management and medication administration
- 7Participate in interdisciplinary conferences and quality improvement initiatives
- 8Perform skilled procedures such as wound care catheter management injections and dressing changes
- 9Coordinate discharge planning and liaise with case managers and community partners
- 10Apply evidence-based practices and stay current with professional development standards
Requirements6
- 1Valid NYS RN license and BLS certification before July 1 2020 or post July 1 2020 with BSN or higher degree
- 25+ years experience in home health nursing with demonstrated expertise in chronic disease management
- 3Proficiency with Spark Airflow and other data analytics tools
- 4Ability to travel independently across NYC using public transportation
- 5Strong communication skills for patient family caregiver collaboration
- 6Commitment to continuous learning through agency provided training and external education
