Community Health Nurse RN, Brooklyn Home Care
Overview
You will own skilled nursing care for patients with chronic conditions across Brooklyn, delivering home care that keeps them out of the hospital. You join a team of 600+ clinicians at NYC Health + Hospitals/Community Care, collaborating with physicians, case managers, and home health aides. This role stands out for its scope: you direct the full plan of care, from admission to discharge, using BLS and evidence-based practices in a community setting.
What You'll Do10
- 1Assess each patient's physical, social, and environmental needs within 48 hours of admission to build a personalized plan of care.
- 2Develop and update nursing care plans with input from the patient, caregiver, and physician, adjusting as conditions change.
- 3Perform skilled treatments including wound care with NPWT/VAC, PEG and trach management, Foley insertion, and JP/PleuRx drain care.
- 4Administer medications safely and teach patients and families about disease and medication management.
- 5Coordinate all services: nursing, rehab, respiratory therapy, nutritional support, and equipment, authorizing as needed.
- 6Supervise home health aides, ensuring proper documentation and quality of care.
- 7Document progress notes, maintain up-to-date patient files, and complete physician orders within 48 hours.
- 8Escalate clinical changes to the physician or provider immediately, preventing complications.
- 9Collaborate with Accountable Care Managers and the interdisciplinary team to plan timely discharges.
- 10Participate in case conferences and quality improvement initiatives to raise care standards.
Requirements9
- 1Valid New York State RN license and current registration.
- 21+ year of experience as a registered nurse.
- 3BLS certification through the American Heart Association.
- 4Bachelor's of Science in Nursing or higher, or obtain within 10 years of licensure.
- 52+ years of full-time nursing experience preferred.
- 62+ years in home care, long-term care, or hospital setting preferred.
- 7Ability to perform comprehensive nursing assessments and skilled interventions.
- 8Knowledge of home care documentation requirements and DOH regulations.
- 9Comfort using computer systems for patient records.
