Registered Nurse (RN) & LPN, Home Care
Overview
You will deliver skilled nursing care in private homes, apartments, assisted living facilities, and nursing homes across New York. This is a Per Diem role with flexible shifts, from one-hour IV infusions to 8-12 hour patient care assignments. You will join a concierge home care team that prioritizes patient preferences and provides 24/7 support. With weekly pay, paid sick time, and holiday pay at 1.5x, you choose the schedule that fits your life.
What You'll Do9
- 1Deliver one-hour IV infusion visits and aide supervision for RNs in patient residences
- 2Provide 8-, 10-, and 12-hour nursing care for patients with chronic conditions, coordinating with families and caregivers
- 3Assess patient vitals, medication schedules, and overall condition, documenting changes in a mobile charting system
- 4Administer IV therapies and medications per physician orders, ensuring sterile technique and patient safety
- 5Develop and adjust care plans for long-term cases, collaborating with the care coordination team weekly
- 6Educate patients and families on disease management, fall prevention, and medication adherence
- 7Respond to urgent changes in patient status, escalating to the on-call nurse when necessary
- 8Document all care provided in a timely manner, meeting state and agency compliance standards
- 9Build rapport with patients and families, ensuring a compassionate and respectful care environment
Requirements8
- 16+ months of nursing experience, geriatric preferred
- 2Valid LPN or RN license in New York
- 3Bachelor of Science in Nursing (BSN) preferred for RNs
- 4BLS CPR certification required for RNs
- 5Malpractice insurance coverage required
- 6Medical clearance: physical, drug test, PPD/Quantiferon within the last year, Rubella and Rubeola titers
- 7COVID-19 vaccination preferred but not required
- 8Ability to commute to Hempstead, NY 11550 for assignment start
