What You'll Do8
- 1Conduct initial and ongoing patient assessments for home health admissions, documenting OASIS outcomes accurately within mandated timelines.
- 2Develop individualized care plans based on clinical findings and patient goals, coordinating with physicians and interdisciplinary team members.
- 3Deliver skilled nursing interventions in the home setting, including wound care, medication management, and patient education.
- 4Document all clinical observations and interventions in the electronic health record with precision and completeness.
- 5Communicate effectively with patients, families, and caregivers, ensuring they understand care procedures and health maintenance strategies.
- 6Coordinate with the CHHA team to ensure continuity of care and compliance with New York State regulations.
- 7Participate in case conferences and quality improvement initiatives to enhance patient outcomes and operational efficiency.
- 8Educate patients and families on disease prevention, treatment compliance, and self-care techniques.
Requirements6
- 1Active RN License in New York.
- 2OASIS experience required.
- 3Graduate of an accredited nursing school.
- 4BSN preferred.
- 51+ years of hospital experience preferred OR 1+ years of CHHA/Home Health experience preferred.
- 6Strong clinical assessment and documentation skills.
