Home Health RN - OASIS & Medicare Compliance
Overview
You will own clinical care for residents in our on-campus home health program, managing full patient cycles from Start of Care to discharge. You'll join an interdisciplinary team in a retirement community setting, coordinating with physicians and specialists to drive outcomes. What You'll Do6
- 1Complete Start of Care assessments, admissions, recertifications, and discharges for home health patients.
- 2Perform comprehensive patient assessments and coordinate care plans with physicians and the interdisciplinary team.
- 3Accurately document using OASIS while maintaining compliance with Medicare and home health regulations.
- 4Monitor patient progress and identify changes in condition requiring intervention or follow-up care.
- 5Educate residents and families on treatment plans, medications, and disease management.
- 6Adhere to all home health regulations, safety protocols, infection control, and mandatory reporting policies.
Requirements7
- 1Current Florida Registered Nurse License (required).
- 2Previous home health nursing experience (required).
- 3OASIS documentation experience and knowledge of home health regulations (required).
- 4Current CPR certification (required).
- 5Strong assessment, communication, and clinical documentation skills.
- 6Knowledge of Medicare regulations and ACHC standards.
- 7Ability to pass a drug test and background check.
Salary Insight
