Senior Case Manager II, Social Services & Veterans Support
Overview
You will lead the case management team at Volunteers of America Los Angeles, mentoring Case Managers and ensuring high-quality services for veterans and families. You will own complex case planning, crisis intervention, and compliance with HMIS documentation. This role blends office administration with field outreach, requiring flexibility and resilience. You will directly shape service outcomes for the most vulnerable populations in Los Angeles.
What You'll Do8
- 1Conduct comprehensive screenings and assessments to develop detailed case plans for clients.
- 2Mentor Case Managers on case load reviews, goal setting, and resource identification.
- 3Coordinate move-in activities, rental negotiations, and referrals to VA and community providers.
- 4Audit case files for completeness, accuracy, and compliance with program requirements.
- 5Lead weekly case conferencing and monthly file audits to maintain quality standards.
- 6Perform crisis intervention for clients facing homelessness, mental illness, or substance abuse.
- 7Develop and deliver workshops on veteran household needs and service access.
- 8Facilitate outreach events to connect vulnerable populations with program services.
Requirements8
- 1Bachelor's degree in social sciences or family therapy from an accredited institution.
- 22+ years of demonstrated case management experience with high-needs populations.
- 3Pass fingerprint clearance and background check, including criminal history and reference checks.
- 4Meet funder approval and maintain current TB screening and immunization records.
- 5Obtain BLS Certification within 15 days of hire.
- 6Complete VOALA Case Management training within 6 months of hire.
- 7Preferred: Master's degree in Social Work or related field.
- 8Preferred: experience in supervision or lead role, and HMIS navigation.
