What You'll Do9
- 1Conduct thorough client assessments and develop individualized service plans addressing housing employment education recovery legal financial medical and mental health needs
- 2Coordinate and monitor services with VA landlords treatment providers and community resources
- 3Provide regular one-on-one meetings and conduct home visits to assess health and welfare
- 4Transport clients in company vehicles to appointments interviews and court appearances
- 5Facilitate life skills and therapeutic groups tailored to veteran populations
- 6Assist with crisis intervention urgent matters and after-hours support
- 7Document services and critical incident reports using MCR and electronic client record systems
- 8Perform random drug and alcohol screenings and ensure compliance with program rules
- 9Support reporting audits and file reviews alongside the Program Coordinator and Director of Behavioral Health
Requirements9
- 1Bachelor’s degree in social work psychology counseling or related field
- 2Two years of case management experience in social services
- 3Ability to pass background checks for programs involving incarcerated or recently released clients
- 4Strong communication documentation and organizational skills
- 5Proficiency in Microsoft Office and electronic records systems
- 6Valid driver’s license with clean record and willingness to complete driver training
- 7Certification in Substance Abuse Counseling preferred
- 8Experience with homeless veteran or justice-involved populations
- 9Passion for veteran advocacy and mission-driven work
