Veterans Case Manager - Homeless Veterans Reintegration Program
Overview
Lead ownership of case management for homeless veterans at HVRP. Manage intake eligibility and job readiness assessments. Develop individualized plans for resume preparation and barrier removal. Ensure participants secure stable housing and employment. Collaborate with employers and maintain detailed case documentation. Drive program success through proactive engagement and strategic partnerships.
What You'll Do11
- 1Conduct intake and eligibility screenings with veterans experiencing homelessness or housing instability
- 2Design and implement individualized plans including resume preparation and mock interviews
- 3Oversee barrier removal processes to guarantee access to essential resources post-placement
- 4Provide ongoing mentorship through regular check-ins via phone or video
- 5Partner with job developers to place veterans in openings with qualified employers
- 6Track progress metrics including enrollment and placement rates across reporting periods
- 7Engage with service providers to build referral networks and expand program visibility
- 8Prepare comprehensive case notes and submit weekly administrative updates
- 9Facilitate weekly one-on-one sessions to monitor professional personal and academic advancement
- 10Collaborate with leadership to refine service delivery models and improve outcomes
- 11Maintain compliance with all regulatory requirements while fostering inclusive workplace culture
Requirements11
- 1Minimum four year bachelor's degree or equivalent experience in related field
- 2One to three years of direct experience supporting workforce development or social services
- 3Proven ability to create personalized service plans for diverse client populations
- 4Familiarity with veteran-specific challenges and resources
- 5Strong understanding of employment services workforce development principles
- 6Proficiency in Microsoft Office Suite and CRM platforms like Salesforce
- 7Excellent written and verbal communication skills with empathetic approach
- 8Ability to manage multiple priorities while meeting strict deadlines
- 9Commitment to supporting underserved communities with dedication and resilience
- 10Certification such as CPC Card or BCBA preferred
- 11Demonstrated track record of achieving measurable results in client outcomes
