Lead Case Manager, Homeless Services & Interim Housing
Overview
Lead Case Manager at PATH leads client engagement across the street to housing continuum within the Hollywood district, guiding a caseload and contributing to operations. You will collaborate with a multidisciplinary team to support individualized housing plans and wraparound services. What You'll Do10
- 1Build and maintain therapeutic relationships with clients to support housing goals and service plan completion
- 2Lead intake and individualized needs assessments and develop ISP addressing housing, income, and wellness goals
- 3Oversee administrative functions and provide coverage for the program manager while coordinating with funders and partners
- 4Support crisis intervention and risk assessment under trauma informed care to enhance client safety and independence
- 5Contribute to program compliance with contracts by coordinating with Quality Assurance and ensuring accurate documentation
- 6Deliver referrals and wraparound services using current community resources to meet client ISP milestones
- 7Conduct 90 day reviews and ongoing progress evaluations with clients and update case files
- 8Use motivational interviewing techniques to support client engagement and problem solving
- 9Generate accurate case notes and maintain complete, current files in accordance with PATH policies
- 10Collaborate with team to ensure continuity of services across housing related programs
Requirements7
- 1Three years of experience in healthcare case management homeless services or working with vulnerable populations
- 2Valid driver’s license reliable transportation and the ability to drive as an essential function
- 3CPR/First Aid training or willingness to obtain prior to hire
- 4Tuberculosis test and background screening required
- 5Drug test required
- 6Employment eligibility to work in the United States
- 7Ability to work onsite in California with standard work hours and possible local travel
