What You'll Do9
- 1Conduct intake assessments and develop Individualized Service Plans aligned with housing and personal goals
- 2Provide referrals and coordinate resources to achieve participant objectives
- 3Complete VI-SPDAT assessments and perform 90-day progress reviews
- 4Maintain HIPAA-compliant documentation and confidential client records
- 5Deliver trauma-informed care using motivational interviewing techniques
- 6Collaborate with cross-functional teams to implement evidence-based homelessness interventions
- 7Perform crisis intervention and risk assessments to enhance client safety
- 8Advocate for systemic change while supporting participants through housing transitions
- 9Utilize data analysis to identify trends and improve program effectiveness
Requirements8
- 1Minimum 2 years experience in healthcare case management or homeless services
- 2Valid driver’s license and reliable transportation
- 3Ability to pass TB screening background and drug tests
- 4Strong analytical skills with attention to detail
- 5Proficiency in communication and relationship-building
- 6Commitment to diversity equity and inclusion principles
- 7Experience with California-specific social service systems
- 8Certification in Crisis Intervention Training preferred
