Case Manager TAY Housing Upward Bound Youth
Overview
The ideal Case Manager & Housing Navigator builds strong relationships with TAY families to secure permanent housing. This role focuses on life coaching and addressing complex needs through positive youth development principles. Responsibilities include developing service plans and coordinating community resources to foster independence and self-sufficiency.
What You'll Do11
- 1Provide life coaching and support services using strengths-based approaches to help youth achieve stability
- 2Develop individualized service and housing plans aligned with youth aspirations and program goals
- 3Assist youth in accessing educational and vocational opportunities through collaboration with specialists
- 4Coordinate transportation logistics using a reliable personal vehicle to support appointment attendance
- 5Conduct clinical assessments and share findings with licensed clinical social workers for follow-up care
- 6Implement trauma-informed practices while delivering crisis intervention and daily support
- 7Facilitate group and individual activities that promote healthy relationships and life skills development
- 8Maintain accurate electronic records in HMIS and prepare progress reports for evaluation purposes
- 9Collaborate with UBH team members and community partners to design engaging programming
- 10Support youth in completing housing applications and credit checks to facilitate permanent placement
- 11Deliver consistent communication and documentation to supervisors and stakeholders
Requirements10
- 1Bachelor's degree in social work or related field with Master's preferred
- 2Minimum two years of direct service experience with homeless individuals especially transition-age youth
- 3Proven experience working with at-risk youth across diverse cultural backgrounds
- 4Demonstrated expertise in mental health, behavioral health, or special education settings
- 5Strong bilingual communication skills English Spanish preferred
- 6Ability to operate a vehicle safely and maintain current driver's license
- 7Background clearance and valid California driver's license required
- 8Excellent written and verbal communication skills with creative problem solving abilities
- 9Experience with data management systems and proficiency in Microsoft Office Suite
- 10Commitment to collaborative teamwork and exceptional organizational skills
