Overview

The Case Manager owns a portfolio of formerly homeless residents and works to sustain housing stability at scale within a contract role in San Francisco. Case management and crisis response skills guide assessments, planning, and referrals to community resources. You will collaborate with onsite staff, property teams, and external providers to support well-being and independence. This role offers a hands-on, mission-driven environment with a focus on practical service delivery and compassionate engagement.

What You'll Do10

  • 1Build individualized service plans and drive housing retention outcomes for residents, coordinating with internal teams to address barriers
  • 2Meet residents at move-in and throughout residency to assess needs, monitor progress, and address concerns
  • 3Coordinate with property staff during unit reviews and habitability follow-ups to resolve apartment-related issues impacting residents
  • 4Connect residents with employment resources, health and behavioral health providers, and other community programs
  • 5Foster a supportive residential environment by organizing tenant meetings and community activities with onsite staff and residents
  • 6Respond to urgent tenant situations using de-escalation techniques and escalate to internal or external providers when needed
  • 7Maintain accurate case records, service documentation, and confidential files in compliance with program standards
  • 8Participate in recertifications, check distribution support, and mandated reporting related to suspected abuse or neglect
  • 9Help residents secure and maintain public benefits, stay current with rent obligations, and remove barriers to tenancy
  • 10Provide crisis support and document incidents to ensure continuity of care and safety

Requirements8

  • 1Bachelor’s degree required, or at least 2 years of verifiable case management experience with homeless or formerly homeless individuals
  • 2At least 1 year of experience working with diverse low-income, homeless, or formerly homeless adult populations
  • 3Hands-on background in case management, outreach, or supportive housing services
  • 4Knowledge of San Francisco resources and service networks for homeless residents with ability to make effective referrals
  • 5Ability to organize, maintain, and update detailed case files for a high-volume caseload
  • 6Proficiency with Microsoft Office and ability to prepare reports, correspondence, and documentation accurately
  • 7Strong problem-solving, interpersonal, and communication skills including crisis situations
  • 8Experience with documentation, filing, and record maintenance in an administrative or human services setting
