Case Manager for Supportive Housing
Overview
As a Case Manager in the Permanent Supportive Housing Department at Pine Street Inn, you will own a caseload of 15-20 tenants, using the Housing First model to drive long-term stability and end homelessness in Boston. You will join an intensive case management team supporting individuals facing mental illness, addiction, and chronic health challenges, collaborating on program-wide stabilization efforts. What You'll Do8
- 1Build individual service plans with tenants, setting goals and tracking progress toward stability within the first 90 days.
- 2Assist tenants with benefits, healthcare, education, employment, and legal aid, ensuring access to critical resources.
- 3Maintain all required program documentation, including case notes and service plans, with accuracy and timeliness.
- 4Develop tenants' daily living skills through coaching and hands-on support, fostering independence.
- 5Respond to crisis situations using effective Crisis Intervention techniques, ensuring safety and coordination with emergency services.
- 6Collaborate with housing staff on program-wide efforts to stabilize all supported tenants, sharing insights and strategies.
- 7Use Harm Reduction and Motivational Interviewing approaches to promote behavioral health and engagement.
- 8Advocate for tenants with local caregiving and advocacy resources, navigating complex systems such as government benefits and legal aid.

- 1High School Diploma or equivalent
- 2Computer literacy with Microsoft Word, Outlook, and Excel
- 3Strong commitment to learning and using Harm Reduction and Motivational Interviewing skills
- 4Knowledge of effective Crisis Intervention techniques and ability to respond in crisis situations
- 5Experience working as part of a team
- 6Preferred: College degree
- 7Preferred: Experience with individuals facing substance abuse, mental illness, trauma, or homelessness
- 8Preferred: Experience with chronically homeless individuals in Housing First programs
- 9Preferred: Knowledge of local caregiving and advocacy resources for people with complex disorders
