Overview
The Case Manager leads supportive housing initiatives at Aeon’s OHP properties in downtown Minneapolis. This role supports 143 formerly homeless adults using a Harm Reduction and Housing First Model. You will manage a caseload of 18 participants while providing personalized case management and connecting residents to essential community resources. What You'll Do11
- 1Maintain a caseload of 18 participants and complete intakes with new applicants
- 2Conduct monthly home visits to assess needs and progress
- 3Develop self-defined goals with participants to foster economic independence
- 4Provide ongoing contact to prevent crises and support housing stability
- 5Coordinate with property managers to resolve tenancy concerns
- 6Arrange transportation for residents to appointments
- 7Assist with housing barriers like document collection and lease education
- 8Secure furniture and household items for resident comfort
- 9Prepare participants for independent living transitions
- 10Establish eligibility for government benefits and financial aid programs
- 11Document cases thoroughly and maintain accurate records
Requirements10
- 1Bachelor’s degree in human services or equivalent experience
- 2Broad knowledge of housing resources and community services
- 3Valid driver’s license and vehicle
- 4Experience with low-income populations and trauma-informed care
- 5Ability to lift 25 pounds occasionally
- 6Strong computer skills in Word Excel and Outlook
- 7Commitment to racial equity and antiracist practices
- 8Excellent communication and teamwork abilities
- 9Experience with Windows-based software and documentation systems
