Housing Case Manager - HSP
Overview
RADIAS Health seeks Housing Case Managers to lead programs in Hennepin and Ramsey counties. The role involves delivering person-centered support to adults with serious mental illness to achieve independent living. This position offers competitive compensation and comprehensive benefits.
What You'll Do11
- 1Provide Housing Support Services to eligible clients ensuring compliance with County State and Medicaid regulations using person-centered approaches
- 2Assist clients with housing applications furnishings and benefit navigation including Cash SNAP HSS and Social Security
- 3Help clients develop budgeting plans and manage Housing Support funds
- 4Teach independent living skills and tenant rights while supporting community resource connections
- 5Coordinate with property management and agencies to resolve housing issues and mediate conflicts
- 6Document client progress and adjust service plans based on ongoing assessments
- 7Schedule and conduct client visits to provide direct support and monitor needs
- 8Intervene during emergencies and communicate changes to supervisors and service teams
- 9Maintain confidentiality and comply with state statutes and HIPAA regulations
- 10Transport clients within the community as needed
- 11Supervise by a Mental Health Professional with access to licensure supervision
Requirements11
- 15+ years building ETL pipelines with Spark and Airflow
- 22,000 hours work experience providing services to adults diagnosed with a serious mental illness
- 3Valid driver's license and reliable vehicle
- 4Experience with case management software and documentation systems
- 5Ability to pass a criminal background check
- 6Strong written and verbal communication skills
- 7Understanding of Minnesota mental health laws and HIPAA compliance
- 8Commitment to person-centered care and advocacy
- 9Willingness to work flexible hours and commute between offices
- 10Previous supervisory experience preferred
- 11Familiarity with Medicaid application processes and local resources
