Overview
RADIAS Health seeks a Mental Health Practitioner to lead housing navigation for individuals with mental illness and substance use disorders in Minneapolis. This on‑site role supports clients in securing independent housing and maintaining tenancy through advocacy and supportive services.
What You'll Do11
- 1Provide instruction and monitoring on service areas outlined in the client’s Housing Focused Person Centered Plan and other clinical recommendations.
- 2Complete Housing Focused Person Centered Plans and modify Housing Support and Crisis Plans as needed.
- 3Communicate service updates to the Clinical Supervisor and multidisciplinary team via phone and email.
- 4Document client progress after face‑to‑face meetings and maintain detailed records of assessments and interventions.
- 5Assist clients in creating action plans to achieve service goals during regular case meetings.
- 6Schedule and facilitate service appointments according to the individualized support plan.
- 7Bill for services using contemporaneous documentation as required by program policy.
- 8Intervene during crises and connect clients with emergency resources, medical providers, and family contacts.
- 9Coordinate care with community agencies and landlords to secure and sustain housing.
- 10Help beneficiaries obtain housing subsidies, economic assistance, and budgeting support.
- 11Transport clients within the community to appointments and services.
Requirements4
- 15+ years experience providing supportive services to vulnerable populations such as individuals with homelessness and co‑occurring mental health or substance use conditions.
- 2Valid Bachelor’s degree in behavioral health sciences or equivalent practical experience.
- 3Proof of vehicle ownership, auto insurance, and a driver’s license.
- 4Ability to commute to the downtown Saint Paul office five days per week.
