What You'll Do11
- 1Conduct comprehensive screenings and assessments to gather functional environmental psycho-social financial employment housing educational and health information for personalized case planning
- 2Provide service linkages and support systems ensuring identified needs are met through coordinated efforts
- 3Coordinate individualized planning with clients to address short- and long-term requirements
- 4Facilitate support groups and client participation when appropriate
- 5Document and chart group education sessions clearly and concisely
- 6Act as advocate for clients before judicial community social service and administrative bodies
- 7Assess client progress regularly and develop detailed plans to achieve goals
- 8Accompany participants to activities and services while facilitating group education
- 9Connect participants to appropriate services as part of care coordination
- 10Maintain mandated reporter responsibilities for abuse incidents
- 11Collaborate with team to manage caseload and deliver case management services
Requirements10
- 1Minimum 1 year experience in case management or similar social services roles
- 2Associate’s Degree in Social Work or related field combined with two years relevant experience
- 3Completion of VOALA Case Management training within six months of hire
- 4Valid fingerprint clearance and background check approval
- 5Strong ability to handle emotionally stressful situations with difficult clients
- 6Proficiency with HMIS navigation systems
- 7Counseling certification preferred
- 8Two years experience working with target population
- 9Understanding of volunteerism and community service principles
- 10Ability to operate effectively in high-stress environments
