Case Aide, Community Outreach & Client Services
Overview
You will own community outreach and client intake for SSG, connecting vulnerable populations to essential services across Los Angeles County. Working under Program Managers and Psychiatric Social Workers, you will screen clients, manage referrals, and track outcomes in databases. You will collaborate with a multidisciplinary team to support care plans and ensure clients receive timely follow-ups. This role stands out for its field-based work, requiring travel to diverse neighborhoods and direct engagement with underserved communities.
What You'll Do9
- 1Conduct education and promotional outreach, distributing program brochures to community partners
- 2Screen potential clients for service eligibility through interviews and documentation review
- 3Handle telephone intake for community referrals, logging details in Outlook and Excel
- 4Connect clients to services outlined in care plans with PSW and CM guidance
- 5Perform follow-up calls and monitor client progress, updating records in the Management Information System
- 6Participate in Care Plan Team meetings to coordinate client support strategies
- 7Maintain and update the project's resource database, adding new community resources
- 8Document client interactions in clinical records and compile monthly reports for PSW and CM
- 9Answer calls and provide appropriate referrals, ensuring callers reach the right services
Requirements8
- 12+ years community work experience or 2 years college coursework in health, social work, or psychology
- 2Knowledge of community resources and referral processes in Los Angeles County
- 3Strong interpersonal skills for diverse and culturally/ethnically varied communities
- 4Proficient in Word, Excel, PowerPoint, and Outlook
- 5Valid California Driver's License and personal insured vehicle
- 6Willingness to work evenings and weekends as needed
- 7Eligibility to work in the U.S. and full COVID-19 vaccination
- 8Ability to work in field settings, including driving in all weather and navigating challenging neighborhoods
