RN Case Manager, Hospice in Minneapolis Metro & Northeast Twin Cities
Overview
Own a dedicated hospice caseload and lead patient-centered care across a localized territory of 5–6 communities. You will coordinate an interdisciplinary team to deliver comfort and quality of life for 10–15 patients. This role offers a consistent Monday–Friday schedule with an on‑call rotation and meaningful impact in patients' lives at Suite Living Hospice Care.
What You'll Do10
- 1Build and maintain individualized hospice care plans for 10–15 patients
- 2Coordinate care across physicians and the interdisciplinary team to ensure timely interventions
- 3Administer medications and provide palliative care to manage symptoms
- 4Educate patients and families on symptom management and care needs
- 5Provide emotional support and advocacy for patients and families during care
- 6Participate in care conferences and family meetings to align on goals
- 7Maintain accurate and timely clinical documentation
- 8Supervise and support hospice aides to ensure quality care
- 9Ensure necessary equipment and supplies are in place for patient needs
- 10Contribute to quality improvement initiatives within the hospice program
Requirements5
- 1Active RN license in Minnesota
- 2Graduate of an accredited School of Nursing
- 3CPR certification
- 41+ year of acute care experience required
- 5Hospice or home care experience preferred
