CNA Hospice Care, Certified Nursing Assistant
Overview
You will own daily personal care for hospice patients across Minneapolis, supporting comfort and dignity in their final days. You join a collaborative team with a dedicated RN Case Manager, a manageable caseload, and on-call support, eliminating the stress of last-minute scrambles. What You'll Do7
- 1Perform personal care activities: bathing, dressing, grooming, toileting, and skin care, ensuring patient comfort and hygiene
- 2Review patient care plans and adjust daily routines to match evolving needs and preferences
- 3Assist with lifting, transferring, positioning, and range of motion exercises, applying safe body mechanics
- 4Support ambulation and physical activity, encouraging mobility within patient limits
- 5Manage dining and nutrition, helping with meals and hydration as outlined in care plans
- 6Document all care and observations in the electronic medical record system, keeping accurate and timely notes
- 7Collaborate with the RN Case Manager and care team, communicating changes in patient status promptly
Requirements7
- 1CNA license in Minnesota, active and in good standing
- 212 months of caregiving experience in home care, assisted living, skilled nursing, hospital, or hospice
- 3Current CPR certification preferred
- 4Strong customer service and communication skills, including active listening
- 5Ability to use electronic medical record systems with accuracy
- 6Physical capability to lift 50 lbs and perform safe transfers with proper body mechanics
- 7Valid driver's license and auto insurance required for daily travel
