CNA Hospice Aide - Kindful Health

As a CNA at Kindful Health, you will deliver hands-on care to hospice patients in Austin, Texas, managing a reasonable caseload with the RN Case Manager. You will represent the care team at IDT meetings every two weeks, coordinating with the Clinical Director and Medical Director to ensure concierge-level hospice service. What You'll Do8
- 1Provide CNA care to hospice patients, following the plan of care set by the RN Case Manager.
- 2Present patient updates at IDT meetings every two weeks, sharing insights from the care team.
- 3Coordinate with the RN Case Manager, Clinical Director, and Medical Director to adjust care plans for best-in-class outcomes.
- 4Adhere to all regulatory requirements and professional standards, maintaining current CNA certification.
- 5Identify system problems or inefficiencies that may affect patient care and report them to leadership.
- 6Manage clinical excellence across your patient census, ensuring CAHPS scores stay high.
- 7Communicate clearly with patients, families, and the internal team to build trust and satisfaction.
- 8Follow policies and procedures consistent with recognized professional practice standards.
Requirements8
- 1CNA certification with a current license, including continuing education.
- 21+ years of hospice or home health aide experience, preferred.
- 3Ability to travel within the local Austin market.
- 4Strong communication skills with patients, families, and care teams.
- 5Demonstrated mastery of hospice care fundamentals.
- 6Willingness to embrace optimism, enthusiasm, and a growth mindset in daily interactions.
- 7Flexibility to work in the field, providing patient care in homes.
- 8High school diploma or equivalent.
Salary Insight
