- 1Adhere to all regulatory requirements and professional standards
- 2Follow policies and procedures aligned with professional practice
- 3Identify system problems or inefficiencies affecting patient care
- 4Manage clinical excellence across your patient census
- 5Provide clear communication internally and externally to improve CAHPS scores
- 6Collaborate with Clinical Director Medical Director and team for concierge hospice services
- 7Maintain accurate documentation and accurate patient information
- 8Participate in regular IDT meetings to share updates and coordinate care
- 9Implement solutions that enhance patient outcomes and family satisfaction
- 10Contribute to team development by sharing knowledge and best practices
- 11Support seamless transition of care between providers and settings
Requirements10
- 15+ years hospice nursing experience in Texas
- 22+ years direct patient care experience in hospice setting
- 3AWS Certified Cloud Practitioner certification preferred
- 4CPC Card credential required
- 5BCBA certification optional but advantageous
- 6Proven ability to thrive in fast-paced environments
- 7Strong analytical skills for identifying process improvements
- 8Excellent written and verbal communication abilities
- 9Commitment to continuous learning and professional growth
- 10Ability to work independently and make sound clinical judgments
Salary Insight
