What You'll Do10
- 1Build and deliver individualized hospice care plans through assessment, planning, implementation, and evaluation
- 2Coordinate with physicians and the interdisciplinary team to ensure seamless, compassionate care and efficient use of resources
- 3Maintain accurate documentation and update patient records to reflect current needs, goals, and interventions
- 4Educate patients and caregivers on medications, mobility, personal care, and end-of-life preparation
- 5Provide emotional and physical support at the time of death and participate in 24/7 care coverage with the team
- 6Attend team meetings, supervise hospice aides, participate in training and special projects, and comply with nursing regulations
- 7Support patient and family education sessions and assist with care transitions and discharge planning
- 8Adhere to safety and privacy standards while managing varied conditions and environments
- 9Demonstrate commitment to AccentCare values and mission while serving as a compassionate role model

Requirements6
- 1Must be a licensed Registered Nurse in the state of practice
- 2CPR certification required in Georgia and California
- 3Prior experience in hospice, home health, medical-surgical, or geriatrics pediatrics; one year preferred in Connecticut
- 4Willingness to serve on-call and uphold the organization’s values and mission
- 5Valid driver’s license, reliable vehicle, and meet minimum state-required auto insurance
- 6Physical ability to perform lifting up to 200 lbs with assistance and to work in varied environmental conditions
