Hospice RN, NJ & Hospice Care Focus
Overview
As a Hospice Registered Nurse you will own patient assessments and care planning for terminally ill patients at scale within the Essex County community. You will work with an interdisciplinary team to ensure a continuous, responsive plan of care from admission through end of life. This role emphasizes compassionate communication with patients, families, and care partners, providing education and emotional support. You will travel to patient locations daily and help deliver dignified, patient-centered care.
What You'll Do10
- 1Build and deliver skilled nursing care per individualized care plans to maximize comfort and quality of life for terminally ill patients
- 2Collaborate with physicians, social workers, chaplains, and nursing staff to develop and adjust care plans
- 3Educate and support families and caregivers on care techniques and coping strategies
- 4Serve as a liaison between patients, families, and healthcare providers to advocate for patient wishes
- 5Coordinate and communicate care needs across the care team to ensure continuity of care
- 6Travel daily to patient locations and facilities as assigned
- 7Document care activities and patient responses to interventions
- 8Monitor for symptom changes and initiate appropriate interventions
- 9Participate in IDT meetings to update care plans and goals
- 10Demonstrate compassionate, customer-service oriented nursing in a fast-paced hospice setting
Requirements6
- 1Valid New Jersey or Multi-State RN license
- 2Minimum of two years of nursing experience in acute-care settings such as medical, surgical, or oncology units
- 3Strong clinical skills with focus on patient comfort and quality of life
- 4Excellent communication abilities and capacity to work independently in a fast-paced environment
- 5Ability to travel daily to patient locations and facilities
- 6Commitment to compassionate hospice care and patient-family education
Salary Insight
$80 - $95k per year
