What You'll Do5
- 1Build and refine hospice care plans based on patient needs and goals to improve comfort within 90 days
- 2Assess patients with advanced illness and manage symptoms to reduce distress and hospital admissions
- 3Educate patients and families about end-of-life options, comfort measures, and care planning
- 4Collaborate with VNA clinicians and external providers to ensure seamless, compassionate care
- 5Document patient status updates and adjust care plans as goals evolve
Requirements6
- 1RN with Bachelor’s degree in Nursing
- 2Certification in Hospice Palliative Care and Critical Care skills preferred
- 31+ year of supervised clinical RN experience
- 4Excellent communication skills
- 5Strong computer skills
- 6Excellent organizational and interpersonal skills
