What You'll Do10
- 1Lead the delivery of compassionate, quality patient care in accordance with physician plans of care and 3HC policies and procedures
- 2Coordinate hospice services across assigned counties and participate in ongoing interdisciplinary assessments
- 3Maintain complete, timely EMR documentation and 48 hour completion of assessment information
- 4Supervise LPNs and certified nursing assistants to ensure CMS Hospice guidelines are followed
- 5Collaborate with Social Workers on bereavement support and assist with care plan development
- 6Communicate with physicians, Hospice Medical Director, and IDG team regarding treatment changes and plan adjustments
- 7Ensure referrals and preauthorization requirements are managed for timely reimbursement
- 8Participate in QAPI and quality improvement activities and contribute to staff education
- 9Rotate on-call duties and cover visits to meet productivity standards
- 10Act as a community resource for Hospice program education and outreach when needed
Requirements8
- 1Registered Nurse currently licensed to practice in North Carolina
- 2Minimum of one-year experience as a professional nurse in an acute care setting
- 3Demonstrated leadership qualities
- 4Experience with terminally ill patients preferred
- 5Strong communication skills with patients, families, staff, physicians, and the public
- 6CPR certification (BCLS)
- 7Valid driver's license and reliable transportation
- 8Ability to perform delegated tasks to support patient outcomes as per care plan
