What You'll Do11
- 1Provide comprehensive outpatient primary care for pediatric adult and geriatric patients managing full-time panels of approximately 1500 patients
- 2Perform physical examinations diagnose acute and chronic conditions interpret lab results and develop individualized treatment plans
- 3Prescribe medications and deliver preventive wellness and chronic disease management services
- 4Educate patients on disease prevention treatment plans and healthy lifestyle practices
- 5Coordinate specialty referrals diagnostic testing and continuity of care
- 6Complete medical documentation disability forms DMV IHSS school DME and related patient paperwork
- 7Manage EHR inbox messages prescription refills and patient communications
- 8Collaborate with physicians advanced practice providers behavioral health professionals nurses and care coordinators within a multidisciplinary care model
- 9Participate in quality improvement initiatives peer reviews clinical audits and organizational performance activities
- 10Provide after-hours on-call coverage on a rotating schedule
- 11Engage in community outreach events and health education initiatives as needed
Requirements8
- 1Minimum 2 years of primary care clinical experience
- 2Medical Doctor MD or Doctor of Osteopathic Medicine DO degree from an accredited institution
- 3Board Certified or Board Eligible in Family Medicine Internal Medicine or related primary care specialty
- 4Current unrestricted California physician license and active unrestricted DEA license
- 5Current BLS certification
- 6Bilingual language skills preferred
- 7Relocation assistance available
- 8Eligibility for HRSA and State Loan Repayment Programs
