What You'll Do9
- 1Diagnose and treat acute and chronic conditions for a panel of 2,000+ patients, using EPIC electronic health records.
- 2Implement evidence-based protocols for diabetes, hypertension, and asthma, reducing readmissions by 15% in the first year.
- 3Coordinate care with specialists, social workers, and community health workers under the Team-Based Care Model.
- 4Lead quality improvement initiatives targeting HEDIS measures, including cancer screenings and immunizations.
- 5Provide telehealth consultations for follow-up visits, improving access for rural and homebound patients.
- 6Mentor resident physicians and medical students during clinic rotations, sharing best practices in family medicine.
- 7Manage urgent care needs within the clinic, performing procedures like suturing and joint injections.
- 8Document all encounters in EPIC within 24 hours, supporting data-driven population health.
- 9Collaborate with pharmacy and behavioral health to address social determinants of health, such as food insecurity.
Requirements8
- 1Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree from an accredited institution.
- 2Valid, unrestricted license to practice medicine in California, with no prior disciplinary actions.
- 3Board certified in Family Medicine or Board eligible with completion of certification within 18 months.
- 42+ years of clinical experience in an outpatient setting, preferably in a Federally Qualified Health Center.
- 5Active BLS certification from the American Heart Association or American Red Cross.
- 6Proficiency with electronic medical records, specifically EPIC, and basic computer skills.
- 7Leadership or management experience in a clinical setting is preferred.
- 8Eligibility for coverage under AltaMed’s malpractice insurance, including education and training requirements.
