What You'll Do10
- 1Build and manage a patient panel delivering comprehensive care across ages 0+ in person and via tele-health
- 2Design and conduct acute, chronic, and preventive visits with measurable outcomes within 90 days
- 3Lead weekly Clinical Rounds and collaborate with in office and virtual teams to optimize patient care
- 4Provide in office procedures within scope of practice and clinical training
- 5Supervise NP or PA colleagues and contribute to continuous care improvements
- 6Educate patients about treatment plans and preventive care using technology tools
- 7Participate in huddles and cross functional team communications to coordinate care
- 8Utilize evidence based medicine and clinical guidelines to improve outcomes
- 9Contribute to a culture of feedback, learning and continuous improvement
- 10Demonstrate readiness to adapt to diverse patient backgrounds and practice settings
Requirements5
- 1Enrolled in, or completed, an accredited Family Medicine or Medicine/Pediatrics residency program
- 2Practiced at least 2 of the last 5 years in outpatient primary care seeing all ages (0+)
- 3Board certified in Family Medicine or Medicine/Pediatrics, or Board Eligible with plans to obtain board certification within 1 year
- 4State license in Texas obtained before start date
- 5Passion for human centered primary care and effective use of technology to deliver care
