Overview
Own the patient journey across a full spectrum of primary care at scale within a team focused on human centered care. Work onsite in Chicago with in person and telehealth visits, ongoing education, and collaboration across clinical and virtual teams. What You'll Do10
- 1Build and manage a patient panel delivering acute, chronic, and preventive care
- 2Lead weekly clinical rounds and collaborate with in office and virtual teams to coordinate care
- 3Supervise NP or PA colleagues and provide mentorship
- 4Deliver in office procedures as appropriate and utilize testing centers for patient care
- 5Maintain high quality care through evidence based practice and use of clinical decision support
- 6Engage in continuous learning and participate in huddles and rounds to improve outcomes
- 7Communicate with diverse patients and families with empathy and clarity
- 8Navigate complex cases with adaptability and teamwork
- 9Participate in telehealth visits to extend care beyond in person visits
- 10Contribute to a culture of feedback and continuous improvement
Requirements5
- 1Enrolled in, or completed, an accredited Family Medicine residency program
- 2Practiced at least 2 of the last 5 years in an outpatient primary care setting
- 3Board Certified in Family Medicine, or Board Eligible with plans to obtain board certification within 12 months
- 4State licensed in IL, obtained before start date
- 5Demonstrated passion for human centered care and ability to communicate across diverse backgrounds
