Family Nurse Practitioner or Physician Assistant, Primary Care at One Medical
Overview
One Medical is seeking a Family Nurse Practitioner or Physician Assistant to join our Chicago clinic. You will own a diverse patient panel, delivering acute, chronic, and preventive care to patients of all ages in a primary care setting. Collaborate with in-office and virtual clinical teams, participate in weekly rounds, and perform in-office procedures as needed. This full-time role is based in Wicker Park, with opportunities for telehealth and multi-state credentialing.
What You'll Do10
- 1Build a patient care plan across acute, chronic, and preventive needs to improve health outcomes
- 2Design and deliver in-office and telehealth visits for patients of all ages
- 3Lead weekly Clinical Rounds and collaborate with in-person and virtual teams to coordinate care
- 4Own in-office procedures and acute care management to support a broad patient panel
- 5Scale patient access through technology and streamlined workflows
- 6Drive continuous learning and quality improvement through huddles and feedback
- 7Partner with peers to ensure state licensure and credentialing for virtual care in additional states
- 8Ship patient-centered care using evidence-based primary care protocols
- 9Develop strong rapport with diverse patients and families to support adherence and satisfaction
- 10Support One Medicals mission to transform healthcare delivery by embracing human-centered care
Requirements10
- 1Completed an accredited FNP or PA program with national certification
- 2In the past 5 years, practiced as an Advanced Practitioner for at least: 2 years in outpatient primary care seeing patients of all ages, OR 1 year plus a primary care fellowship or 1+ year in urgent care seeing patients of all ages
- 3State license in IL obtained by start date
- 4Demonstrated passion for human-centered primary care and ability to communicate with diverse patients
- 5Proficiency using technology to deliver high quality care
- 6Clinical proficiency in evidence-based primary care
- 7Willingness to obtain additional state licensure and credentialing for virtual care in other states
- 8Ability to work collaboratively in a team and adapt to changing patient needs
- 9Commitment to ongoing feedback and professional development
- 10Reside or work in Illinois with readiness for in-person clinic duties at the Wicker Park location
